Step 1: Schedule an appointment
Schedule an appointment with your primary care physician or podiatrist. If you are diabetic, make sure to bring this medical necessity form with you to your visit with your primary care physician

Step 2: Bring us your information
Bring in your insurance information, prescription and letter of medical necessity (if diabetic) along with medical records from your visit. We'll take care of the rest!

Step 3: Insurance approval
Depending on your insurance, you will be able to receive your shoes same day or you may have to wait for your insurance to grant us authorization. This can take anywhere from 1-3 weeks

Step 4: Enjoy your new shoes
Once the authorization comes through, we will give you a call to come back and pick out your shoes. You can choose from a variety of styles that we have in store or we can always order you any pair you want!
